Enterobiasis

Enterobiasis that is pin worms infestation causes more nonsense than any other worm but it is said to be the least obstinate type and the most easily curable infestation. Still the patient suffered and persistently demanded treatment for 18 yrs.  but when got correct homoeopathic medicine  not only relieved but felt so good that she stopped taking medicine and almost one year has passed when she is trouble free.

Diagnosis – Enterobiasis / oxyuriasis.      
H/o Treatment - The patient has taken several courses of mebendazole, pyrantel palmoate etc. in last 10 yrs. with temporary relief.
Complaints  –
-          Constipation since early hood.   Stool scanty, hard, small like sheep dung, yellow, stick together with                       
            mucus and Pin warms.
-          Urge - 8- 10 times a day since 10 years as she remembers.
-          Pin worms and anus burning, itching more at night since 10 years.
-          Itching vulva. Frequency of urination sometimes.
-          Appetite increased. Eats several times in a day.
-          Nose - Since more than one & half year sneezing and watery discharge in morning  for one & half hours, aggravates in cooler, when brooms or dusting.
-          Face – Pimples & white heads.
-          Thirst less.
-          Desires meat & fish, salt.
-          Adverse milk & milk products.
-          Perspires on upper lip, nose and chin (face), Axillae.
-          Chilly, always sleeps with cover takes blanket even during summer nights.
-          Takes hot both in winter at 2-3 days interval.
-          Sleep-Deep. Lies on abdomen. Talks during sleep.
-          Dreams frequent, pleasant, of day events.


-          Anger violent, averse of being lone in house but with to be alone in her room.


-          Fears in dark, climbing down, height. 
-          Weight 42 kg. Hight-5'.
-          Patient was born normal of delayed labor, suffered gastroenteritis at age 14 months.
-          Mother has sciatica, hypotension, oral ulcers & lock jaw due to gutkha habit .
-          Father has  anxiety neurosis, hypertension.
-          Brother suffers from tonsillitis.  Had pulmonary tuberculosis.

Mother’s statement –Patient was constipated since infancy and had pinworms in early child hood .  Mile stones were normal. She is very demanding for her proper treatment.
-          Patient menarche age 14 years.  Menses late 10 to 15 days but flow for 7 days, profuse for first 3 days.
-          Lumber back pain 1 day before menses.
-          Irritability during menses. Averse to work during menses.

           -         Anal Itching was  subsided third day onward but recurred since three days.
-          Constipation unchanged
-          Appetite decreased toward normal but feels internal trembling if does not eat.
-          Nasal discharge and sneezing -0
-          Chilliness unchanged, must cover at night
-          Weight – 43 ½ k.g.

-          Bowel moves once daily.  Worms recurred for last few days.
-          Menses on 15 th   September .Was trouble free.
-          Covers at night in bed, says more out of habit !!
-          Appetite- normal.
-          Acne face, itching.
             -         Weight 44 kg.

After this prescription patient did not reported and stopped medication. Her father informed me that she was o.k. and has no complication or pin worm.

KIDNEY STONES

Kidneys are two bean shaped organs of size of fist situated in loin both sides of vertebral column.   They filter out the excess and the waste material from the circulating blood, in the form of urine.  Through ureter the urine is carried to urinary bladder where it is stored until passed by the person. The amount of urine passed per day depends on the intake of fluids and fluid lost in sweating & breathing.    
Kidney stones are pieces of solid material formed out of substances in the urine.  Stone may stay in kidney or travel down the ureter or urinary bladder.  It may stick anywhere in urinary tracts and causes severe pain and can block the flow of urine. Symptoms of kidney stones and their severity may vary from person to person and according to site and size of the stone.  Ø  Severe pain in kidney or ureter is felt in kidney region or abdomen moves to lower abdomen or groin. Pain may last for minutes to hours.
Ø  Nausea and vomit.
Ø  Burning urethra and / or bladder with frequent urge to urinate.
Ø  Dark or dirty urine, mixed with blood.
Ø  Blocked flow of urine.
Stone may stays in kidney without pain for years and it keeps growing, affecting the functions and structure of the kidney.  They may cause repeated episodes of pain, blood in urine, blocking the passage they cause kidney swollen-filled with urine.  There then results  hypertension due to renal damage.
Why Kidney stones form
Ø  Inadequate fluid intake.
Ø  Chronic or recurrent urinary tract infection.
Ø  Certain drugs.
Ø  Urinary tract obstruction as due to  some enlarged blood vessel.
Ø  Limited activities.
Ø  Certain genetic and metabolic diseases.
Type of stones - Four types of stone are defined by there composition.
· Calcium stones - These are calcium compounds oxalate or phosphate.  Calcium stones are the most  common type of stones and the oxalates.
· Struvite stones - These are formed after an infection. 
· Uric acid stone forms in an excessively acidic urine.   It is more common in  meat eater.
· Cystine stone - These occur rarely but it runs in family. Cystine is a sulphur containing amino-acid helps forming healthy hair, skin, bones, connective tissue and digestive enzymes.

Treatment Kidney stones  ® It includes pain management and remedial treatment of stone. If there is pain attack an antispasmodic pain killer should be given.
-          Fluid intake should be increased more than two & half liter i.e,12-15 glasses of water a day.
-          Most stones pass with in 48 hours if sufficient fluid is taken. A stone of 4 mm has 80 %
           chance while that of size 6 mm has only 20 % chance of passage. Generally a stone sized
           above 6 mm does not pass.   Such stones need further treatment.
-          The most important is to maintain fluid intake more than two and a half liters a day.   
-          There are homoeopathic medicines help dissolving and passing a stone while some relieve
            pains.
-          Diet - In calcium stone should be low in sodium and oxalates so avoid spinach and leafy
           vegetables, pumpkin, tomatoes, beets, nuts, chocolates, coffee, tea, colas, strawberries,
           wheat bran. In uric acid stones avoid animal protein, eat less meat, fish and poultry also
           legumes, mushrooms, cauliflower, yeast.
         
Medicines for pain relief
-          Colocynthes Q, Dioscorea Q, Scutelaria Q
-          Ocimum  canum Q or 30 potency
-          Magnesia phosphorica. 6x
-          Coffea 0/3, Chamomilla 0/3, Belladona 0/3 are good pain killer when prescribed for indicated
           symptoms.

1.       Some medicine are preferably effective on stone  in right kidney and some on left while some
          are effective when stones are formed in both kidneys.
           ·         Stones in both kidneys - Berberis vulgaris Q, Hydrangia Q, Ammi visnaga Q, Taraxacum
                      Q, Althea Q, Ulmus rubra Q,  Pareira Q, Zea mays Q.
           ·         Stone in Right kindney - Sarasparilla Q, Equisetum - Low potencies, Calcarea renalis 3x,
                     Lycopodium 30, Nux vomica 30, Ocimum canum -3x, 6x, or 30
           ·         Stone in left kidney – Berberis velgeris Q, Hydrangia Q, Zea mays Q, Pareira Q.
2.       Ureteric stone ®  When stone gets lodged in ureter, gives a lot of troublesome symptoms.
          Few medicines help to ease the passage of stone through ureter are Valeriana off Q, Ammi
          visnaga Q, Hydrangia Q, Ocimum can Q or 30, Hygrophilla augustifolia Q or 30, Acid phos Q.
3.       The other troublesome symptom besides the pain is burning urination for which Cantheris 30
           can be added. Some time the pain is accompanied with nausea, vomiting, vertigo, sweating &
           anguish   -Tabacum 30 will relieve the condition.  If with pain there is frequent urge for stool
           or constipation with crampy pain - Nux vomica 30 can relieve it.
4.       Hematuria – i.e. blood in urine during & after pain needs attention, Hammamelis Q,
          Millifolium Q, Uva ursi Q, Phosphorus 30 or 0/30, Erigeron Q, Solidago Q or Terebinthina 30
          may help.5. Aloe vera Q, Hydrangia Q, Zea mays Q, calcarea renalis 3 x or 200, Berberis
          vulgaris high potencies may prevent recurrence of kidney stones.
Beside homoetopathic medicines surgery is another option for removal of renal stones.  It should be considered only if stone-
-          Does not pass after a reasonable period of time and causes constant pain.
-          Does not dissolve and too large to pass or caught in a difficult place.
-          Blocks the flow of urine, causing ongoing kidney infection.
-          Damaging kidney tissue or causing constant bleeding.
-          Has grown larger.
The various surgical options are-
1.       Extracorporeal Shock Wave Lithotripsy (ESWL)- In  this procedure measured shock waves are
          passed through the stone to break it in to tiny pieces in urine.
          -          Most patients would have hematuria for few days after lithotripsy with minor
                     discomfort in back or abdomen.
          -          Sometimes broken stone particles may cause pain and discomfort when passing
                      through urinary track.
          -          Sometimes stone may not be completely shattered with one treatment.
          -          ESWL is not recommended for very large stones.
2.       Percutaneous Nephrolithotomy -  The stones in kidney or in the upper half of ureter are
          removed by surgical means.
3.       Ureterscopic stone removal - Some stones in ureter can be treated with ESWL, but
          ureteroscopy is  useful for stones in middle or lower ureter.

Here are some CASES of STONES in urinary tract responded dramatically under homoeopathic treatment.
1.       Mr. T. A., 39 yrs. U.S.G. dated 16/05/10 – A calculus of size 24.8 mm is seen at the renal
          pelvis of left kidney. Calcarea carb.10 M  one dose weekly, Calcarea renalis 3x thrice a day,
          Berberis vul.Q 10 drops thrice a day with water. Tribulus t. Q 10 drops thrice a day with water.
          Zea mays  Q 10 drops thrice a day with water and Profuse fluid intake was prescribed. Result
          were as follow -
USG – 17/06/10 Left renal calculi 5.8, 6.1 and a renal pelvic calculus of size 22 mm seen with
                            hydronephrosis .
USG – 23/07/10 A calculus size 17.3 mm is seen left renal pelvis with hydronephrosis.
USG – 2/09/10  Left kidney – multiple calculi of size 7.13mm and 6.65mm seen in pelvic region and
                           a 5mm calculus seen in lower calyx.
2.       Mr. G. S. 62 yrs. C/o Stranguary, pain urinary bladder. Constipation.  
USG 04/07/10 – Right Kidney – moderate Hydronephrosis, few calculi size 4 -7 mm in middle
                            calyx, Right hydroureter due to a calculus size 11mm at 5 cm distal to rt. Pelvi-
                            ureteric junction. Left kidney – a calculus size 6 mm seen in upper calyx.
Sulphur – 200 one dose followed by Calcarea renalis 4x thrice a day, Berberis vul. Q  10 drops twice a day,  Acid Phosphoric 30 thrice a day, Colocynthese Q 10 drops with water during pain as and when require.
 12/08/10  USG - Right Kidney – 6.1 mm calculus and few crystals seen. Left Kidney – 5.4 mm calculus and few crystals seen.
The medicines continued  with symptomatic relief.
11/09/10  Pain lumber region. USG – Right Kidney - 5.7 and 6.3 mm calculi seen. Left Kidney  -  5.4 and 4.7 mm calculi seen. Sulphur 200 one dose, Calcarea phos 30 thrice a day, Berberis vul. 30 thrice a day and Hydrangia Q twice a day
12/10/10  Lumber back pain off and on. USG - Left Kidney – 6.1 mm calculus seen. Prostate – 25.6 gms. The medicines were repeated.
3.  Mr. R. N. (Marble Trader) 30 Yrs. Hindu Male Married    Vegetarian.
Diagnosis – Reno-uretero-vesicular calculi.
USG. 12/7/10  Kidneys- Bilateral small calculi, Rt. Ureter – 7.2 mm calculus at Rt. Ureterovesicular junction hydroureteronephrosis, Vesicular calculus – 16 mm.             
H/o Acute lumber back and abdomen pain – 2 month back. Urine – stopped – 7th   June (due to calculus in bladder). Lt. abdomen pain in a line since one month. Bowel unclear during pain in back. Thirst increased.  Sweating increased.  Desire salty food.
Natrum Mur. 200                Two doses at 10 minute interval every week, Acid phos. 30 Two pills thrice a day. Ocimum canum Q 10 drops Thrice a day with a half cup of water. Berberis vul.     Q 10 drops thrice a day, with a half cup of water, Colocynth + Dios Q for pain –10 drops in a half cup of  water thrice a day or as required during pain.         Magnesia phos 6x for pain 4 tablets thrice daily or as required during pain.
29/7/10        No pain. Same medicines were repeated.
27/8/10 No pain. Bowel – better. USG- Both kidney calculi size 4.6 mm. Urinary Bladder calculus size 6.9 mm. Continued same medicines.
19/9/10  Constipation. Lumber back pain aggravates afternoon. Weariness.                      
Aesculus H. 200 two does at 10 minute interval every week, Acid phos. 30 two pills three times a day, Ocimum can Q and Berberis vul Q 10 drops each thrice a day with a half cup of water. Magnesia phos 6x 4 tablets thrice daily. H.R.O. to apply on back before sleep.

Research Question
Still many questions are to be answered - How we can screen & protect the person at risk from getting stone?  Why some people continue to get painful stones?  What are long term effects of lithotripsy? Do genes play roll in stone formation?  What are natural substances in urine which prevent stone formation?
